Please note that an employee can tell their employers that they have been off sick for work for up to 7 days without needing a note from their GP – this is called ‘self-certification’ (you can find out more by visiting the Gov UK website at: https://www.gov.uk/government/publications/statutory-sick-pay-employees-statement-of-sickness-sc2 ).

This form is specifically to request a continuation of a MED3 certificate, commonly referred to as a “sick note”. This facility should only be used AFTER a triage with a GP has taken place. The practice will not take this submitted forms as a request for consultation, instead please contact reception to book a triage with a GP to discuss.

Ensure that you complete all steps of this form to successfully request a sick note (MED3). The more information you can provide us with, the more helpful this is for our admin team to process the request through to our Doctors and avoid delays.

Please allow 48-72 hours for the practice to consider and process your request.

Agreement to issue the sick note requested is at the complete discretion of the Doctor.
